This case involves a couple in their early 40s who typically attended face-to-face counselling to address recurring communication challenges and relational tension. On this particular occasion, they were unable to attend in person because their daughter was unwell with COVID-19, and the babysitter naturally did not want to assist. Concerned about the potential risk of infection, they also wanted to avoid exposing the therapist or our therapy room to any health risks. They had a strong pattern of attending sessions regularly, rarely cancelling, and so using online counselling ensured continuity and prevented disruption to their progress. Couples counselling in this format allows clients to maintain a consistent therapeutic connection even when attending in person is not possible.

While we generally prefer face-to-face therapy for local clients, this situation highlighted the importance of flexibility and client-centred care. The couple had a strong pre-existing therapeutic relationship with their therapist, which enabled a smooth transition to the online format. Their trust in the process and the rapport already established meant that meaningful therapeutic work could continue effectively despite the change in modality.

During the online session, the therapist maintained a close and attuned connection with the couple and responded to subtle emotional cues, just as would have happened in a face-to-face setting. The therapist guided the partners in noticing their own reactions, tuning into each other’s emotions, and responding thoughtfully during challenging exchanges. Real-time guidance from the therapist helped the couple remain present and regulated, ensuring that emotionally charged discussions were productive rather than overwhelming.

The session drew on a combination of therapeutic approaches, demonstrating how techniques usually applied in face-to-face settings can be adapted effectively online. Psychodynamic exploration helped the couple identify recurring relational patterns rooted in past experiences and family-of-origin dynamics. Cognitive Behavioural strategies were used to highlight unhelpful thinking patterns and develop alternative responses during conflict. Somatic awareness and mindfulness exercises supported the couple in attuning to bodily responses and staying present during emotionally triggering interactions. Experiential techniques, including structured dialogue and role-play, were also incorporated to enhance insight, empathy, and relational connection.

By the end of the Online Counselling session, the couple reported feeling heard, understood, and less frustrated. They valued the continuity of therapy and appreciated that the online format allowed them to maintain safety while still engaging harmoniously. They finished the session with practical strategies to manage ongoing communication challenges, including mindful pauses, reflective listening, and noticing physiological cues that signal tension.

This case illustrates how Online Counselling can be an effective alternative when in-person sessions are not feasible, particularly when a strong therapeutic relationship already exists. Many therapeutic approaches including psychodynamic, cognitive-behavioural, somatic, mindfulness, and experiential techniques, can be adapted successfully for online use. Flexibility in delivery allows clients to continue their therapeutic work without disruption, maintain safety, and receive personalised, high-quality care regardless of external circumstances.
