This case involves a woman in her mid-50s who sought mindfulness-based therapy to manage chronic stress and anxiety following a recent diagnosis of breast cancer. Prior to her diagnosis, she had led an active lifestyle, balancing a demanding career in project management with family responsibilities. Despite her outward resilience, she had long experienced internal pressure to “do it all,” often neglecting her own needs in favour of others. The cancer diagnosis intensified her stress, heightening worries about health, mortality, and the impact of her illness on her family. She sought therapy to develop strategies for staying present and reducing the overwhelm that accompanied her daily life.

From the outset, it was evident that she had a tendency to become absorbed in future-oriented worry, replaying worst-case scenarios, and catastrophising potential outcomes. This pattern not only heightened anxiety but also made it difficult for her to engage fully in daily activities or connect with supportive relationships. She described feeling “constantly on edge” and finding it challenging to pause, rest, or enjoy the present moment. These patterns, while exacerbated by her health situation, had roots in lifelong tendencies toward perfectionism, self-criticism, and hyper-responsibility.

Mindfulness therapy focused on cultivating awareness of internal states and external experiences in the present moment. Early sessions involved guided attention exercises, breath awareness, and gentle body scans, helping her notice tension, restricted breathing, or emotional agitation without judgment. She was encouraged to observe her thoughts as passing events rather than truths requiring immediate action or solution. Initially, this was difficult, as she was accustomed to “fixing” or controlling everything around her, including her own emotions.

As therapy progressed, she began applying mindfulness practices during daily routines, pausing before responding to stressful emails, noticing bodily sensations while walking, and bringing attention to small moments with her family. She became increasingly able to identify early signs of stress, such as shallow breathing, tight shoulders, or racing thoughts, and use grounding techniques to reduce tension. This allowed her to respond to challenges with greater clarity and less emotional reactivity. She also noticed an increased ability to tolerate uncertainty related to her health, observing fears without becoming consumed by them.

A pivotal moment occurred when she described a particularly anxious episode before a medical appointment. Using mindfulness strategies, she focused on her breath, acknowledged her fear without judgment, and remained present during the consultation. Afterwards, she reflected on the experience as empowering: rather than being swept away by anxiety, she could engage fully in the moment, notice her responses, and act with intention. This marked a significant shift in her relationship to stress and uncertainty.

By the later stages of therapy, she reported feeling more grounded, present, and compassionate toward herself. Daily mindfulness practices had reduced overall anxiety, improved sleep quality, and enhanced her ability to participate in family and social activities. She described a growing sense of agency, recognising that while she could not control the progression of her illness, she could influence her response and maintain connection to life’s meaningful moments.

This case illustrates how Mindfulness can help clients with chronic stress cultivate present-moment awareness, observe thoughts and emotions without judgment, and develop practical strategies for emotional regulation. By integrating Mindfulness into daily life, clients can reduce anxiety, strengthen resilience, and reclaim a sense of balance and agency even in the face of challenging circumstances.
