This case involves a 60-year-old woman who wanted to learn Mindfulness to support her management of chronic pain associated with rheumatoid arthritis. She had experienced ongoing joint inflammation and stiffness for several years, which significantly affected her mobility, daily routines, and overall quality of life. She described cycles of frustration, fatigue, and emotional tension that often accompanied flare-ups, noting that pain not only impacted her body but also her mood and confidence in daily tasks. She sought support to reduce the emotional impact of pain, increase her ability to remain present in her body, and develop practical strategies for self-care.

From the outset, it was clear that her experience of pain was amplified by stress and anticipatory worry. She reported feeling tense even during periods of lower inflammation, bracing her body in expectation of discomfort. This heightened physical tension often increased pain sensations, creating a reinforcing cycle that contributed to fatigue, irritability, and difficulty maintaining normal activity levels. She also described challenges with sleep, restlessness, and mental preoccupation with symptoms, all of which intensified her emotional distress.

Mindfulness interventions were introduced to help her cultivate awareness of her body and emotional states without judgment. Early sessions focused on guided attention exercises, such as noticing subtle physical sensations, acknowledging discomfort without resistance, and observing fluctuations in pain with curiosity rather than fear. Through these practices, she began to distinguish between the raw sensation of inflammation and the emotional amplification that often accompanied it. Learning to observe rather than immediately react to pain provided her with a sense of agency and reduced the intensity of her stress responses.

Gentle Bioenergetics exercises were integrated alongside mindfulness practices to support mobility, release muscular tension, and enhance body awareness. Movements were tailored to her capacity, emphasizing slow, deliberate actions coordinated with breath awareness. She was encouraged to notice areas of tightness or soreness during each exercise, accepting these sensations as information rather than obstacles. This combination of mindful attention and gentle, intentional movement allowed her to engage with her body in a supportive, non-judgmental way, reducing the fear of triggering flare-ups while improving circulation and flexibility.

A pivotal moment occurred during a guided session when she noticed habitual tension in her shoulders and back. Rather than attempting to “push through” the discomfort, she paused, adjusted her posture, and brought attention to her breath. Over several repetitions, she observed a significant decrease in perceived tension and a newfound sense of spaciousness. This experiential insight reinforced the principle that mindful presence and gentle, intentional movement could directly influence her perception of pain and her bodily responses.

Over the course of therapy, she reported improvements in physical comfort, emotional regulation, and quality of life. Mindfulness helped her break the cycle of anticipatory tension and reactive stress, while gentle exercise promoted mobility and body awareness. She was able to maintain daily activities with less fear of pain, approach flare-ups with greater calm, and integrate Mindfulness into routines such as stretching, walking, and rest periods. Importantly, she described feeling more connected to her body and more confident in her capacity to manage symptoms independently.

This case illustrates how Mindfulness combined with gentle movement practices, can support clients living with chronic somatic conditions. By cultivating present-moment awareness, recognising bodily signals without judgment, and integrating compassionate movement, clients can reduce emotional amplification of pain, improve function, and enhance overall well-being.
